MEDICAL EDUCAITON IN HARYANA

                        Medical education and its curriculum is basically dependent upon general education system and type of health care system in a country as a whole. The medical education status of a state will depend upon general policies which are framed at national level.

A. GENERAL EDUCATION IN HARYANA

The existing system of general education in our country has developed to a great extent but it has failed to deliver the desired effects of education. As a result of this a large number of people in Haryana are deprived from getting education. The literacy rate of 55.85% is the testimony to the above statement. On the other hand the economic development has been still faster, but the gainers are few.

The education that is being taught in the institutions has little connection with the day to day functioning of life. It is not helpful for greater number of people in making them self confident and self reliant to lead a better life. In such circumstance it is not surprising that scientific awareness is very much lacking which in turn becomes a great rider for new scientific innovations to be adopted for the development of the society. Continuance of higher education in non-professional stream has become mostly an alternative for unemployment. Even the science subjects are loosing their hopes. New subjects like computers, commerce and M.B.A. have limited number of seats though they have a temporary career for students.

In the present system of education there is little scope for assessing the aptitude of students which could direct their choice for vocational training or professional education. For higher studies merit has become synonymous to money. It depends upon the paying capacity of the parents that which area the student will follow.

Because of various reasons, most of the students cannot continue their studies beyond eighth, so very few of them get the eligibility to complete for vocational, professional or higher education. Even if a student quality he cannot compete with students coming from an affluent family and trained in better institutes. Even the competition is becoming stronger amongst the latter. Corruption and manipulations are being reported in newspapers as the major mode of getting into a carrier. Admissions, examinations and services in Haryana are reported to be manipulated through all petty means.

B.  HEALTH CARE

Every country has got its own health culture. The present system of health care which was introduced by our foreign rules didn’t care of the health needs of our people in general because they wanted only to impart some treatment to their own people and to those who could some to their proximity. The Indian system of medicine was made to stagnate as there were prejudices against it. Ultimately it got perverted also. Obviously such a neglected system could not compete with the modern medicine.

PRESENT HEALTH DELIVARY INFRASTRUCTURE OF HARYANA

 If we compare the health infrastructure at the time of independence and now, one can say that lot of progress has been made but a lot more is desired to be done. The following tables give some insights of health care delivery system in Haryana.

            The present medicare system has given triple benefit to the vested interests.

1.       It has distracted the attention of the people at large from demanding the basic prerequisites of good health which is food, shelter, potable water, disposition of human and animal excreta and education. In other words of social justice is being denied. This has become very clear from effects of new economic policy in health sector.

 

2.      It has helped the vested interests in using these institutions to keep social and political control over the weaker sections making them subservient to the stronger section to beg relief which otherwise should have been their right to obtain.

3.      It has helped them in using this scientific and technological advance for their commercial benefit i.e. enormous profit at the cost of human health and life.

To achieve this sociopolitical and economic benefit the dominant social classes have introduced a type of medical education which is bound to serve the interests of these classes at the cost of people’s health at large. This is the story of the last 50 years. 

 

PRESENT STATUS OF MEDICAL EDUCATION IN HARYANA

            There are two medical colleges, four dental colleges and three ayurvedic colleges in Haryana. Few more are being opened. Students coming from upper strata of society get admission in medical colleges. Naturally they shouldn’t have social orientation and commitment to serve the society at large. Students are not selected on the basis of aptitude because of the commercial benefit paradigm.

            Present medical education is institutionalized and is dependant on sophisticated investigations and therapies. The approach towards health problems is predominantly unifactorial although it is a reality that health and disease are basically multifactorial. The irony is that people are aware of the ill effects of the unifactorial approach but don’t dare to change it because:

a.    those who are entrusted to change it and are capable of doing so are the products of these institutions and also the beneficiaries.

b.   training itself is misdirected and actively obstructs the scope of viewing the patient in totality i.e denying the multifactorial approach of medicare. Doctors and paramedical personnels thus become agents of commercial benefits of the planners. The planners have introduced the recipients in order to create a market so that persons responsible for medicare don’t rise against this system because of their competitive interests.

POLICY LEVEL CHANGES

1. Change in the general education:      

            It is suggested that to implement a people oriented, need based medical education, one should change the present system of general education. The opportunity must be opened to all sections of our society so that the children from the lower strata get an opportunity to qualify of higher education (General, Vocational and Professional). For this the change in the timings of the schools, creative and innovative methods of teaching, change in the curriculum of general education, well oriented teachers etc are some of the essential components.

PROPER SCREENING OF STUDENTS

            There must be screening to divert the students to different streams depending on their aptitude and performance by continuous assessments by the teachers and the parents as well. This will need a reorientation of outlook of the teachers and also the parents. This is again very much dependent upon the job opportunities and scope of employment in respective fields. It is understandable that in mixed society like India this is difficult to implement but it is time that we demand for it and change the direction of present system of medical education.

               LOPSIDED PRIORITIES SHOULD CHANGE

            Present policy has created a situation in Haryana where more doctors are being produced in medical institutions without giving proper attention to the training of paramedical staff such as nurses, anganwadi workers, lab technicians and pharmacists. We need nurses and paramedics in large numbers. We need specialists for our community health centres which are starving because of lack of specialists. Medical colleges which are being planned to be opened in private sector will increase this disparity further as the seats for nurses courses are being reduced.

            Thrust areas in special reference to specific health problems in Haryana e.g. residual effects of pesticides on human beings, urinary system stones, psychiatric problems, infant mortality rate, maternal health and gender problems are some of the areas which need more attention.

EFFICIENT MANAFEMENT OF THE INFRASTRUCTURE

            Health care is primarily the responsibility of the state. Individuals have a role to play for care of his health but the individual can’t go beyond the influence of their cultural, socio-political and economic environment.

            The staff in CHC’s PHC’s and Sub centres should be adequate. The necessary software and hardware should be made available in these centres. Public accountability of the persons working should be fixed. Required infrastructure should be provided as per the norms laid down.

            There should be properly regulatory law for private nursing homes in Haryana.

            The burden on PGIMS, Rohtak is mainly from Rohtak district (about 60%). The serious and problematic cases referred from the other districts may not get required attention because of increased work load. PGIMS, Rohtak is having CHC Dighal, Civil Hospital Beri, CHC Kathura (Chiri) and CHC Chhara under its direct administrative control. A bilateral referral system can be started on experimental basis between Rohtak District and PGIMS, Rohtak. The patients from Rohtak city first should go to Civil Hospital Rohtak and from there they can be referred to PGIMS, Rohtak. This may improve the functioning at PGIMS, Rohtak also.

            REORIENTATION OF MEDICAL EDUCATION IS REQUIRED

            Students must be selected purely on basis of aptitude and performance assessed during general education through continuous assessments. The objective of medical education should be to prepare:

A.    Basic doctor who will form the base for health care for the people.

B.     Medical Specialists

C.     Medical Scientists

D.    For optimal use of new technological advancements by the medical institutions to avoid exploitation of the poor patients by private sector and to teach students.

E.     Nurses and paramedical staff as per the required norms.

There shouldn’t be any capitation fee.

TRAINING CURRICULUM

A.    Selected students should be posted for 6 months in primary health centers to reorient them to the socioeconomic and cultural needs of Haryana. There itself they will learn basic anatomy, physiology and pathology.

 

B.      After 6 months they should go to sub divisional / district / regional hospitals for further one year under specialists of different disciplines to get training. Further detail of anatomy, physiology and pathology in relation to the disease under treatment will be taught.

 

C.     After eighteen months of training the students will be taught in medical institutions for next three years to learn basic science and clinical disciplines through intensive and extensive teaching with well adjusted curriculum so that the course will be for and a half years as it is at present.

 

D.    There should be continuous assessment and monitoring of the students. This will help them screen for basic doctors, medical specialists or medical scientists. Students are also mature enough to plan their future at this stage. Medical scientists for basic sciences need not get intensive training for clinical medicare after obtaining their degrees. Medical specialists will be trained extensively in their respective specialties. Internship after graduation should be there for one year at CHC level.

 

The three years extensive training at medical colleges should be there to strengthen to conceptual basic of different specialties. Specialist training after A, B and C should be encouraged depending upon the requirements of Haryana.

 

WHAT SHOULD BE THE CURRICULUM

 

In context of present scenario, there is globalization with open market economy which has created a group of people (about 10 crores in India and twenty laksh in Haryana), who can complete U.S.A, U.K and Japan in all aspects and their expectations are definitely very different than rest of the people (about 85 crores) who either can’t make their both ends meet or are surviving with great difficulty.

More rational approach will be to plan the curriculum of medical education which can respond to the needs and expectations of the people at large. Keeping in above things in mind, the curriculum can have following elements:

1.      Standard text books on all subjects authored by Indian doctors must be prescribed.

2.      Applied aspects of basic science – 18 months – should be scheduled for the field for which a new curriculum should be developed.

a.       First aid management in general and of particular diseases should get required emphasis.

b.      Basic knowledge about these problems should be imparted.

c.       Area specific diseases should be taught.

3.         Theoretical details should be taken up at PGIMS Rohtak in next three years.

4.         Honours course ( extra paper ) for those who want to become medical specialists or medical scientists should be undertaken ( computer course should be part of this curriculum ).

5.         In undergraduate period emphasis on social and preventive medicine should form a major part.

6.         Learning by doing alongside the bed of the patient in the field should be the approach of teaching. Rational portions of other system of medicines should become part of curriculum with a concept of “modern medicine”

7.         Region specific curriculum:

a.       Farm accidents and road accidents.

b.      Residual effects of pesticides in human beings.

c.       Disturbing trends in sex ratio and its implications.

d.      Urinary tract stones etc.

8.      Exposure to super specialties:

The basic principles of super specialties should be undertaken.

 

EXAMINATION SYSTEM

 

A.    Uniform pattern

B.     Major credit should be given to continuous assessment.

C.     More emphasis should be given to clinical diagnosis and treatment.

D.    Theoretical knowledge may be assessed by written and practical exams at the time of final examinations.

E.     For advance courses separate examinations should be held.

The conscious efforts should be made in this direction so that some concrete policy level decision can be taken.

वसुधैव कुटुंबकम

 वसुधैव कुटुंबकम के दर्शन को जी-20 के सामने रखना होगा

      जी-20 के इंडोनेशिया शिखर सम्मेलन के बाद रोटेशन क्रम में भारत को जी-20 देशों की अध्यक्षता करने का मौका प्राप्त हुआ है। जी-20 के हर एक सदस्य को रोटेशन के हिसाब से अध्यक्षता कम करने का मौका बारी-बारी से खुद-ब-खुद मिलता रहेगा। यह अध्यक्षता किसी भी देश की उपलब्धियों को देखकर नहीं, बल्कि रोटेशन के हिसाब से मिलती है।
      हमारे प्रधानमंत्री ने भारत द्वारा जी-20 की अध्यक्षता ग्रहण करने के बाद, "एक पृथ्वी, एक कुटुंब, एक भविष्य" की थीम के इर्द-गिर्द एक घरेलू राजनीतिक प्रचार अभियान छेड़ने की घोषणा की है और ऐलान किया है कि जी-20 की भारत की अध्यक्षता एकता की सार्वभौमिक भावना को बढ़ाने का काम करेगी। इस मुद्दे को लेकर वह पिछले दिनों भारत की समस्त राजनीतिक पार्टियों की एक बैठक भी कर चुके हैं। यही पर सबसे जरूरी सवाल उठता है कि भारत ऐसा क्या करें कि उसकी नीतियां और मंतव्य दुनिया के सामने पहुंचें?
      इसमें पहला काम होगा कि भारत अपने देश में ऐसे समाज और राजनीतिक ढांचे का निर्माण करें कि जो सबको समानता, समता, आजादी और न्याय मुहैया कराये, जहां जाति धर्म वर्ण रूप रंग नस्ल और क्षेत्र के आधार पर भेदभाव न किया जाए और संवैधानिक गारंटियों को अमलीजामा पहनाया जाये।
      दूसरे, हमें संसार को दिखाना होगा कि वसुधैव कुटुंबकम की भावना के तहत हमारे देश में मौजूद, सभी विविधताओं का आदर और सम्मान किया जाए और पूरी दुनिया में ऐसी सोच और भावना से काम किया जाए।
      तीसरे, गरीबी तथा बेरोजगारी के डरावने तरीके से बढ़ते स्तर के साथ-साथ हमारे देश में आर्थिक मंदी डरावनी और गहरी होती जा रही है इसका पूरी तरह से इलाज करके, जनता की हिफाजत किया जाए की जाए और सांप्रदायिक ध्रुवीकरण के साथ बढ़ती हुई हिंसा, आतंक और नफरत पर कड़ी नकेल कसी जाए।
      चौथे, जिस तरह से महिलाओं, दलितों, आदिवासियों और हाशिए पर पड़े हुए करोड़ों लोगों के साथ सामाजिक, आर्थिक और राजनीतिक अन्याय के मामले, दिन दूनी और रात चौगुनी की गति से बढ़ रहे हैं, इन पर प्रभावी अंकुश लगाया जाए और जिस तरह से संवैधानिक गारंटियों और असहमति की तमाम अभिव्यक्तियों को "राष्ट्र विरोधी" बताकर जिस तरह का सलूक किया जा रहा है, उस पर तत्काल रोक लगा कर दोषी लोगों के खिलाफ कठोर से कठोर कार्रवाई की जाए।
      पांचवें, जिस तरह से जनतांत्रिक अधिकारों और संवैधानिक गारंटियों को गंभीर तरीके से कतरा और चोट पहुंचाई जा रही है, उस पर तुरंत प्रभावी रोक लगाई जाए।
       छठवें, आजादी समानता समता न्याय और भाईचारे के हमारे जो संवैधानिक अधिकार स्तंभ, हमारे सार्वभौमिक, धर्मनिरपेक्ष, जनतांत्रिक, समाजवादी और गणतांत्रिक राज्य को परिभाषित करते हैं, उन सब पर मजबूती से कायम रहा जाए।
      सातवें, सरकार द्वारा जिन उद्देश्यों और सोच की घोषणा की गई है, उसे यथार्थ में बदलने के लिए आर्थिक, सामाजिक और राजनीतिक क्षेत्रों में मिल रहे डरावने रुझानों को बेहद मजबूती के साथ, जनता की भागीदारी को साथ लेकर अविलंब सुधारा जाए।
      आठवें, भारत को अपने देश की जनता और पूरी दुनिया को यह बताना जरूरी है कि भारत को, पिछले 30 साल की जनविरोधी, निजीकरण, उदारीकरण और वैश्वीकरण की नीतियों को छोड़कर और तत्काल त्याग कर, जनकल्याण की नीतियां, दुनिया की जनता के सामने रखी जाएं और पूरी दुनिया को बताया जाए कि प्रत्येक देश के साथ-साथ दुनिया की जनता का कल्याण निजीकरण, उदारीकरण और वैश्वीकरण की नीतियों से नहीं हो सकता। प्रत्येक देश की जनता की जरूरतों के हिसाब से वहां की नीतियां बनाई जाएं।
       उपरोक्त जरूरी कदमों को तत्काल से तत्काल उठाकर ही, भारत दुनिया की राजनीति सोच, नजर और नजरिए को सही दिशा दे सकता है। ऐसा करके ही भारत जी-20 का अध्यक्ष बनने के नाते, देश और दुनिया की जनता के हित में काम कर सकता है और उसके अंदर जनकल्याण की नई मानसिकता सोच और नजरिया को पैदा कर सकता है और इसी बहाने से भारत अपनी "वसुधैव कुटुंबकम" और "विश्व बंधुत्व" की सोच से पूरी दुनिया को अवगत करा सकता है।

 एमएसपी की  क्रांतिकारी मांग

आज यह साफ है कि पूरे कृषि समाज का संकट कृषि पण्यों के बाजार की परिस्थितियों से पैदा हुआ संकट है। किसान को अपने उत्पाद पर लागत जितना भी दाम न मिल पाने से पैदा हुआ संकट है। अर्थात एक प्रकार से किसानों की फसल को बाजार में लूट लिए जाने से पैदा हुआ संकट है। और जो इजारेदार अब तक इस बाजार को परोक्षतः  प्रभावित कर रहे थे, अब उनके खुलकर सामने आकर कृषि समाज पर अपना पूर्ण अधिपत्य की कोशिश का सीधा परिणाम है यह प्रतिरोध संघर्ष।
     कृषि क्षेत्र में भूमि सुधार ही हर समस्या की रामबाण दवा नहीं है। किसान को जीने के लिए जोतने की जमीन के साथ ही फसल का दाम भी समान रूप से जरूरी है। भारत के किसान आंदोलन को भी हमेशा इसका एहसास रहा है। इसलिए तो एमएसपी को सुनिश्चित करने की मांग किसान आंदोलन के केंद्र में आई है।
  सन 2004 में स्वामीनाथन कमेटी के नाम से प्रसिद्ध किसानों के बारे में राष्ट्रीय आयोग का गठन भी इसी पृष्ठभूमि में हुआ था जिसने अक्टूबर 2006 में अपनी अंतिम रिपोर्ट में कृषि पण्य पर लागत के डेढ़ गुना के हिसाब से न्यूनतम समर्थन मूल्य को तय करने का फार्मूला दिया था । तभी से केंद्र सरकार पर उस कमेटी की सिफारिशों को लागू करने का लगातार दबाव है । यह तो साफ है कि यदि कृषि बचेगी तो उसके साथ जुड़े हुए ग्रामीण समाज के सभी तबकों के हितों की रक्षा भी संभव होगी।
      सरकारें कुछ पण्यों  पर एमएसपी तो हर साल घोषित करती रही, एपीएमसी की मंडियों वाले दो-तीन राज्यों के किसानों को थोड़ा लाभ भी हुआ। लेकिन लागत के डेढ़ गुना वाला फार्मूला सही रूप में आज तक कहीं भी और कभी भी लागू नहीं हुआ है और न ही एमएसपी से कम कीमत पर फसल को न खरीदने की कोई कानूनी व्यवस्था ही बन पाई है। जब किसान अपनी इन मांगों के लिए जूझ ही रहे थे कि तभी अंबानी-अडानी की ताकत से मदमस्त मोदी ने बिल्कुल उल्टी दिशा में ही चलना शुरू कर दिया। और इस प्रकार एक झटके में पूरी कृषि संपदा पर हाथ साफ करने के इजरेदारों के सारे इरादे खुलकर सामने आ गए।
     आज इस किसान आंदोलन ने देश के हर कोने में पहले से बिछे हुए असंतोष के बारूद में जिस प्रकार एक पलीते की भूमिका अदा करनी शुरू कर दी है , उससे इस ज्वालामुखी के विस्फोट की शक्ति का एक संकेत मिलने पर भी कोई भी इसका पूरा अनुमान नहीं लगा सकता है।

 यहाँ मैं एक बात और स्पष्ट कर दूँ कि डॉ. आंबेडकर जी का ऋण केवल एक वर्ग है ऐसा नहीं है। महिलाओं के अधिकारों की उनकी लड़ाई प्रत्येक भारतीय महिला के लिए थी फिर चाहे वह किसी भी जाती विशेष की हो। स्वतन्त्र भारत का व्यापारी वर्ग यदि आज सारे भारत में व्यवसाय करने का अधिकार रखता है (कुछ उन परिस्थितियों में नहीं रखता जब राज्य सरकारें कुछ व्यवसायों की वृद्धि करने के लिए इस अधिकार को सीमित कर देती हैं) तो वह भी इसलिए है क्योंकि डॉ. आंबेडकर जी ने संविधान में ऐसे प्रवधानों की रचना की और ज़ोर देकर उन्हें संविधान में रखवाया। आज यदि भारत के एक राज्य का नागरिक स्वतंत्रता से किसी दूसरे राज्य में जा कर नौकरी या व्यवसाय कर सकता है तो यह भी इसलिए है कि संविधान के निर्माण के समय डॉ. आंबेडकर जी ने ही ऐसे प्रवाधानों को ज़ोर देकर संविधान का हिस्सा बनवाया क्योंकि उनका मानना यह था कि यदि एक राज्य से दूसरे राज्य में जा कर काम करने की स्वतंत्रता नहीं होगी तो इससे लोगों में अखंड भारत की भावना को ठेस पहुंचेगी। आज भारत के किसी राज्य में यदि अत्याधिक उपद्रव फैलता है तो केंद्र के पास बल प्रयोग का अधिकार है। यह भी इसलिए है क्योंकि डॉ. आंबेडकर जी ने ऐसे प्रावधान को ज़ोर देकर संविधान में रखवाया था। उनका मानना था कि यदि केंद्र के पास यह शक्ति नहीं होगी तो इससे भारत में उन लोगों के निजी स्वार्थों पर नियंत्रण रखना मुश्किल हो जाएगा जिनसे अखंड भारत के स्वरूप को खतरा पहुँच सकता है। ऐसे अनगिनत उपकार डॉ. आंबेडकर जी के भारत के हर नागरिक पर हैं फिर चाहे वह किसी भी जाती या धर्म से हो। आप डॉ. आंबेडकर जी को अपना उद्धारकर्ता माने या न माने, वही आपके उद्धारकर्ता हैं यह इतिहास के पन्नों में दर्ज है। आप उनके इस ऋण को स्वीकारें या नहीं स्वीकारें पर आप उनके ऋणी हैं यह इतिहास के पन्नों में दर्ज है। आप उन्हें पढ़े या न पढ़े पर आनेवाले समय में आपके भले के लिए ही वह लिख कर गए हैं, यह भी स्पष्ट है।


तो, आपको यह समझना होगा कि आप कैसे बौद्धिक विकास कर सकते हैं। डॉ. आंबेडकर जी को पढ़ना इस कार्य के लिए एक पर्याय है क्योंकि इससे आप उस श्रेष्ठ व्यक्ति को पढ़ेंगे जिसने आपके लिए इतना कुछ किया। उनकी सोच आपमें सामाजिक विचारों को पैदा करेगी। उनके बौद्धिक विकास से आपका बौद्धिक विकास भी होगा। उन्होंने परेशानियां झेली और मुक्ति के मार्ग खोजे। उनके मार्ग आपके काम भी आ सकते हैं। उनके उपाय निजी नहीं थे बल्कि सबके हित में थे इसलिए आपको भी ऐसे उपाय नज़र आएँगे जो आपका और औरों का विकास कर सके। उनका ज्ञान भविष्य के लिए है। वह आपके आनेवाले कल के लिए हैं। आपको आनेवाले कल में गुलामी से मुक्ति दिलाने के लिए। आपकी संतानों को बौद्धिक सम्रद्धि देने के लिए। जिन्होंने भी डॉ. आंबेडकर जी को पढ़ा उनके परिवार में बौद्धिकता का प्रवेश हुआ है। यह एक सत्य है। आपको भी इस सत्य को खोजना है। तो फिर प्रण लें कि आप लतों से दूर होने के लिए बौद्धिकता का विकास करेंगे और डॉ आंबेडकर जी को  जरूर पढ़ेंगे।  - जय भीम, निखिल सबलानिया

 अब नोट छोड़ कर डॉ. भीमराव आंबेडकर जी को पढ़े और बुद्धिजीवी बनें -


पिछले सात वर्षों से मैं डॉ. भीमराव आंबेडकर जी के विचारों का प्रचार कर रहा हूँ। इसी कड़ी में मैंने पांच वर्षों पहले डॉ. आंबेडकर जी की पुस्तकें भी बेचनी प्रारम्भ की। हालाँकि मैं फिल्म और टीवी में कार्यरत था फिर भी मैंने अपने जीवन का एक बड़ा समय डॉ. आंबेडकर के विचारों को प्रचारित करने में लगाने का निश्चय किया। इसलिए मैंने उनके विचारों पर कई वीडियों का भी निर्माण किया जो आप यूटयूब के चैनल Ambedkartimes पर देख सकते हैं। पर यह अफ़सोस की बात है कि पांच वर्षों के इस कार्य में महज दो हजार लोग भी ऐसे नहीं होंगे जिन्होंने आगे बढ़ कर डॉ. आंबेडकर जी के साहित्य को पढ़ने के लिए खरीदा हो।

मेरे मन की व्यथा न केवल एक सच्चे आंबेडकरवादी की व्यथा है, बल्कि समाज और देश का भला सोचनेवाले एक सच्चे व्यक्ति की व्यथा है। अपना अच्छा कैरियर त्याग कर समाज के लिए भलाई सोचना अपने लिए गढ्ढे खोदने के समान है भले ही यह एक आदर्श कार्य है। इस दौरान मैंने देखा कि प्रत्येक वर्ष लाखों अनुसूचित जाती के छात्र-छात्राएं कालेजों में डॉ. भीमराव आंबेडकर जी के दिलाए आरक्षण के द्वारा दाखिले लेते हैं पर उनकी पुस्तकें नहीं पढ़ते ? लाखों युवक-युवतियां सरकारी नौकरियों के लिए दाखिले के लिए तो पढ़ते हैं और आरक्षण से दाखिला लेते हैं पर आरक्षण दिलानेवाले अपने पिता डॉ. आंबेडकर जी की पुस्तकें नहीं पढ़ते। राजनीती में आरक्षण से नेता बनने वाले, चुनावों में तो लाखो-करोड़ों रूपये खर्च कर देते हैं पर, आरक्षण दिलवानेवाले बाबा साहिब डॉ. आंबेडकर जी के साहित्य को नहीं पढ़ते। यहॉं तक कि जो लोग गैरसरकारी नौकरियों या व्यवसाय में लगे हैं वे यह नहीं सोचते कि यदि बाबा साहिब डॉ. आंबेडकर जी उन्हें कानूनी रूप से समता का अधिकार नहीं दिलवाते तो अपने परंपरागत धंधों (जिनमें तुच्छ कोटि के कार्य शामिल थे) को छोड़ कर वह दूसरे कार्य भी नहीं कर पाते। आज भारत में जो स्त्रियां प्रोपर्टी पा कर अच्छा जीवन व्यतीत कर रहीं हैं, वह भी नहीं सोचती कि उन्हें यह अधिकार बाबा साहिब डॉ. भीमराव आंबेडकर जी ने दिलवाया। उन्हें शायद यह भी नहीं पता कि महिलाओं को संपत्ति का अधिकार दिलवाने के लिए उन्होंने कानून मंत्री के पद से इस्तीफा दे दिया था। यह अधिकार न सिर्फ कुछ वर्गों की महिलाओं को ही प्राप्त हुआ बल्कि भारत में सभी वर्गों और जातियों की महिलाओं को प्राप्त हुआ। पर भारत की महिलाऐं भी डॉ. आंबेडकर जी को नहीं पढ़ती।

पिछले दिनों नोटबंदी हुई और पूरा देश हाहाकार मचाने लगा। गरीब और मध्यम वर्गीय इसलिए परेशान हैं कि उन्हें लाइनों में घंटों लगना पड़ रहा है और अमीर इसलिए परेशान हैं कि काले धन को सफ़ेद कैसे बनाया जाए। मैं भी बहुत से विचारों से इस स्थिति का मंथन करता रहा। हालांकि मेरी स्थिति बहुत से लोगों से अलग है। जब लगभग सभी लोग नोटों और धन की अंधी दौड़ में व्यवसायी वर्ग द्वारा दिखाए गए सपनों की तरफ भाग रहे थे तब मैंने समाज के लिए कार्य करने को चुना। मेरे मन में बहुत बार ऐसी बातें आईं कि मैं जो कर रहा हूँ क्या यह सही है। एक किस्सा आपको अपने जीवन का बताता हूँ।

एक बार मैं एक बड़े से बैग में डॉ. आंबेडकर जी की पुस्तकें एक ऐसे प्रकाशक से खरीद कर लेकर जा रहा था जो कि लगभग बंद हो चुका है। यह प्रकाशक डॉ. आंबेडकर जी की और जाति की समस्याओं पर पुस्तकों का प्रकाशन करता था। पर पाठक न मिलने के कारण इसे प्रकाशन बंद करना पड़ा। बैग भरी होने के कारण मुझे उसे उठाने में बहुत मेहनत करनी पड़ रही थी। जिस रस्ते से मैं जा रहा था वहाँ सोने और हीरे के बड़े सुनारों की दुकाने थीं। मेरे मन में बात आई कि मैं जो यह बोझा घसीट कर ले जा रहा हूँ और इसे लोगों को बेचने के लिए खूब परिश्रम करूँगा तब कहीं जा कर उतना भी नहीं कमा पाऊंगा जितना कि यह सुनार एक छोटे से हीरे या चंद ग्राम सोना बेच कर कमा लेंगे। लोग भी हीरे और सोने ही अधिक खरीदते हैं। मुझे समझ नहीं आया कि कौन सही है। समाज की भलाई का सोचनेवाला मैं या समाज से मात्र धन कमानेवाले यह सुनार। आप ही बताइए कि सोने और हीरे से समाज का भला हो सकता है या डॉ. भीमराव आंबेडकर जी के विचारों से। अफ़सोस तो यह है कि आज जो अनुसूचित जाती, जनजाति या पिछड़े वर्गों के लोग हैं,  वह भी ज़रा-सा धन आने के बाद इन्हीं सोने और हीरों के पीछे भागते हैं न कि डॉ. आंबेडकर जी की पुस्तकों के पीछे। उन्हें धन चाहिए विचार नहीं।  उन्हें  उत्पन्न किए गए आविष्कार चाहिए, पर वह अविष्कारक की अवहेलना करते हैं। उन्हें डॉ. आंबेडकर जी के समता वाले समाज में अवसर तो चाहिए ,पर उनसे प्राप्त हुए फल को वह स्वयं तक ही सीमित रखना चाहते हैं।

एक दूसरा किस्सा बताता हूँ। मेरे पास ऐसे कुछ लोगों के फोन आए जो कि डॉ. आंबेडकर जी को पंजाबी, उर्दू और सिंधी भाषाओं में पढ़ना चाहते थे। कुछ पुस्तकें मुझे पंजाबी में मिली जो कि एक मित्र पंजाब से नई दिल्ली रेलवे स्टेशन पर ले कर आए। वहाँ उन्होंने मुझसे पूछा कि यह पंजाबी पुस्तकें हम पंजाब में नहीं बेच पाते तो आप दिल्ली में कैसे बेचोगे ? मैंने उन्हें कोई जवाब नहीं दिया क्योंकि मैं बेचने की भावना से नहीं बल्कि इस भावना से ओतप्रोत था कि मुझे तो बस पंजाबी में पढ़नेवाले लोगों तक डॉ. आंबेडकर जी को पहुँचना है। उस दिन रात के तकरीबन दस से गयारह बजे के बीच में पुस्तकों का बहुत ही भारी बक्सा कन्धों पर उठा कर नई दिल्ली पर बने आखरी प्लेटफार्म से पहाड़ गंज के पहले प्लेटफार्म तक लाने में सर्दी में भी मेरे पसीने छूट गए। अफ़सोस यह है कि दो वर्षों से अधिक समय होने के बाद भी इक्का-दुक्का पंजाबी की पुस्तकें बिकी हैं। 

यही नहीं पंजाबी की और पुस्तकें और साथ में उर्दू की पुस्तकें भी मैंने डॉ. आंबेडकर फाउंडेशन से, बेचने के लिए खरीदी। उस समय फाउंडेशन के एक अधिकारी ने कहा कि आप पंजाबी और उर्दू में नहीं खरीदिए यह बिकेगी नहीं। पर मैंने तब भी इसलिए ही खरीदी क्योंकि मेरा मकसद पुस्तकें बेचना नहीं बल्कि डॉ. आंबेडकर जी को पंजाबी और उर्दू पढ़नेवाले लोगों तक पहुँचना था। पर आज तक एक उर्दू की पुस्तक भी नहीं बिकी है। और पंजाबी की पुस्तकों के भी इक्का-दुक्का सैट ही बीके हैं। यह तब है जब कि मैंने दिन-रात लगा कर डॉ. आंबेडकर जी की पुस्तकों का खूब प्रचार किया है। फेसबुक, ट्विटर, वाटसएप, हाईक, जीमेल, ब्लॉग, इंस्टाग्राम, यूट्यूब, क़्विकर अदि ऐसा शायद ही कोई माध्यम छूटा होगा जिससे मैंने डॉ. आंबेडकर जी की पुस्तकों की जानकारी दिन-रात लोगों तक न भेजी हो। पर अफ़सोस कि लोग उनमें रूचि ही नहीं दिखाते। पर आज मुझे इस नोटबंदी से एक आशा की किरण यह नज़र आ रही है कि लोगों का दिमाग पैसे से हट कर किसी दूसरी जगह लगेगा।

कल ही मैंने अपने एक मित्र से कहा कि अधिकांश्तर लोग शूद्र हैं। यह शूद्र हैं क्योंकि यह अपने को व्यवसाईयों, राजनेताओं और धार्मिक गुरुओं पर छोड़ देते हैं। इन शूद्रों में अधिक पढ़े-लिखे और कम पढ़े-लिखे और अनपढ़, सभी आते हैं। इनमें कम आय वाले और मध्यम आय वाले भी आते हैं। यह धन के ज़रा से स्रोतों को पा कर चैन से सो जाते हैं। नौकरीवाला रोज श्रम करने वाले की इसलिए परवाह नहीं करता, क्योंकि उसे लगता है कि उसकी नौकरी उसे प्रत्येक माह एक निश्चित आय जरूर दिलवा देगी। भले ही उसकी आय के लिए वह रोज मजदूरी करनेवाला श्रमिक जिम्मेदार हो। रोज मजदूरी करनेवाला धन का संचय कम करता है और यह नहीं सीख पाता कि कैसे वह मजदूरी छोड़ कर व्यवसायी बने। लोग सोचते हैं कि जीवन में एक बार आई. ए. एस. या कैसी भी सरकारी नौकरी की परीक्षा पास कर लें और उनकी जीवनभर की टेंशन समाप्त हो जाए। पर वह यह नहीं सोचते कि सरकार के लिए धन को अर्जित करने के लिए व्यवसायी और श्रमिक दिन-रात लगे रहते हैं।  नेताओं के लिए खुद को भगवान की स्थिति में रख कर, लोगों पर अत्याधिक टैक्स डालना आसान है, पर वह स्वयं कोई व्यवसाय नहीं कर सकते और मजदूरी तो छोड़ ही दो। जहाँ तक बैंक में कार्य करनेवालों की बात है तो यह और कोई नहीं बल्कि पुरानी सहुकारी व्यवस्था का ही एक रूप है और एक ऐसा व्यर्थ कार्य जिससे कि फिजूलखर्ची बढ़ती है। भविष्य में बैंकों की संख्या बहुत कम हो जाएगी। भारत के अधिकांश्तर लोग शूद्र हैं क्योंकि वह मानसिक गुलाम हैं।

भारत के लोगों को बहुत सी चीज़ों का मानसिक एडिक्शन हैं, या कहें कि ऐसी बहुत सी चीजों की लत (व्यसन) है जिनसे ये स्वयं की मानसिक स्वतंत्रता दूसरों को दे देते हैं। इससे इनमें बौद्धिक क्षमता का विकास नहीं हो पाता। बौद्धिक क्षमता का विकास न हो पाने की वजह से ये स्वयं, परिवार, समाज और देश एवं दुनिया के बारे में सोच नहीं पाते। इससे इनमें रचना शक्ति और नवीन विचारों की कमी रहती है। और यदि कोई नवीन विचार उत्पन्न भी होते हैं तो ये उनसे दूर भागते हैं। बौद्धिक विकास की कमी ने इनकी काल्पनिक शक्ति को भी कमजोर कर दिया है।  काल्पनिक शक्ति की कमजोरी के कारण न तो इनमें वैज्ञानिक सोच ही उत्पन्न हो पाती है और न ही ये समस्याओं के सही समाधान ही निकाल पाते हैं। इन बौद्धिक असक्षमताओं की वजह से ये सही निर्णय भी नहीं ले पाते। गलत निर्णय इनके जीवन को और बुरा बनाते हैं और इससे इनमें नकारात्मक (नेगेटिव) सोच अधिक उत्पन्न होती है।

अपनी इन मासिक लातों के शिकार हुए लोग अपनी बौद्धिक क्षमता खो देते हैं। यह लतें जो कि नशे, धर्म, व्यसनों, लालसाओं में लुप्त रहना जैसे स्वर्ण आभूषणों, और हीरों की लालसा, विलासपूर्ण जीवन की लालसा, महंगे कपड़े पहनना, महंगी करें खरीदना आदि, राजनीती, मनोरंजन (अत्यादिक फ़िल्में और टीवी देखना), खेलकूद, चाय, काफी या कोला जैसी नशीली खाद्य सामग्रियों का सेवन आदि से प्रेरित होती हैं, धीरे-धीरे लोगों के जीने का मकसद बन जाती हैं। उनके जीना का यही उद्देश्य रह जाता है कि केवल अपनी इन लातों को संतुष्ट करना। और आज यह लतें जिनके द्वारा लोगों द्वारा लगाई जाती है वह लोग सिर्फ और सिर्फ धन की लालसा से प्रेरित हो कर सब में कोई-न-कोई लतें लगवातें हैं। और वह चंद लोग जो लोगों में लतें लगवाते हैं वह लोगों के भले की नहीं सोचते।  उनका उद्देश्य समाज, देश या दुनिया का भला करने का नहीं होता। वह लोगों का बौद्धिक विकास नहीं करना चाहते। उनका उद्देश्य लोगों का बौद्धिक ह्रास (विघटन, नाश) करने का होता है। वह निरंतर लतों को प्रचार प्रसार करके आपको उनका आदि बना देते हैं।  और उन लालसाओं की पूर्ति के लिए आपको केवल और केवल धन ही नज़र आता है। और अंततः आपके जीवन का उद्देश्य बौद्धिक विकास न करके अत्याधिक धन का संचय करना ही रह जाता है जिससे आप अपनी उन लतों को संतुष्ट कर सकें। 

आपकी एक लत को आप पूरा करते हो और आपको लगता है कि आपको संतुष्टि मिल गई। परंतु ऐसा करके आप संतुष्ट नहीं होते बल्कि आपकी बौद्धिक क्षमता का जो ह्रास (नाश) होता है उससे आपकी लत और सुदृढ़ हो जाती है। या कहिए कि आपकी मानसिक गुलामी और बढ़ जाती है। व्यक्तिगत बौद्धिक क्षमता का ह्रास (नाश) एक परिवार का ह्रास (नाश) है। एक परिवार का ह्रास पूरे समाज का ह्रास है। और समाज के ह्रास से देश का ह्रास है। क्या आज हमारे देश का ह्रास नहीं है ? क्या हमारे समाज का आज ह्रास नहीं हो चुका है ? क्या हमारे परिवारों का ह्रास नहीं हो चुका है ? क्या आज हमारा व्यक्तिगत ह्रास नहीं हो चुका है ? और देखिए उन लोगों को जिन्हें हमने अपनी मानसकि स्वतंत्रता दी। देखे व्यवसाईयों को, नेताओं को, सरकारी कर्मचारियों को, खेल और मनोरंजन जगत के लोगों को, बैंकों के कर्मचारियों को, धार्मिक गुरुओं को, नशे के सौदागरों को, जिन पर हमने अपनी मानसिक गुलामी दी। देखिए कि हमने स्कूल और कालेजों को आदर्शवादी माना पर यह संस्थाएं भी धराशायी हो चुकी हैं क्योंकि शूद्र जनता और धूर्त लोग, दोनों ही इन्हीं संस्थाओं से निकलते हैं।  यह संस्थाएं न तो बौद्धिक क्षमताओं का ही विकास कर पाती हैं और न ही सही प्रकार नैतिकता दे पाती हैं।

अब क्या है आपके हाथों में ? चंद हजार रूपये ? क्या इनसे आप अपनी उन लतों और उन व्यसनों को दूर कर सकते हैं जो धूर्त लोगों ने आपको लगाई हैं।  इसलिए मैंने प्रारम्भ में कहा कि आज नोटबंदी में मुझे आशा की किरण नज़र आती है। मुझे उम्मीद है कि भले ही नोटबंदी का यह काल (समय) कितना ही छोटा हो, फिर भी यह काफी समय होगा कि लोग अपनी इन लतों को छोड़ कर कुछ और सोचें। लोग सोचेंगे तो उनमें बौद्धिक क्षमता बढ़ेगी। वह किसी अंधे की तरह धन के पीछे नहीं भागेंगे। उनमें उन साधुओं का विवेक उत्पन्न होगा जो सांसारिक वस्तुओं का मोह भांग कर लेते हैं। उनमें उन सिद्धार्थ गौतम की वे आध्यात्मिक लालसा उत्पन्न होगी जिससे वह संसार के प्रलोभनों को छोड़ कर बुद्ध बनने ने लिए निकल पड़े थे। इसलिए मैंने यह भी कहा कि डॉ. आंबेडकर जी को पढ़ना जरूरी है क्योंकि जो पिछड़ा तबका आजतक शूद्र ही है वह आखिर क्यों नहीं उठ पाया ? वह सामाजिक, राजनैतिक और आर्थिक रूप से पिछड़ा हुआ है।  क्योंकि वह डॉ. भीमराव आंबेडकर जी के दिलाए आरक्षण से स्कूलों में दाखिला और छात्रवृति तो चाहता है, वह कालेजों में आरक्षण से दाखिला तो चाहता है, वह आरक्षण से सरकारी नौकरी तो चाहता है, वह संविधान में समता के अधिकार से गैर-सरकारी नौकरियों और व्यसाय में लगना तो चाहता है, वह आरक्षण से नेता और मंत्री तो बनाना चाहता है, वह (महिलाएं) संपत्ति में अधिकार तो चाहती हैं, पर यह सब दिलवानेवाले बाबा साहिब डॉ. भीमराव आंबेडकर जी के विचारों को कोई पढ़ना नहीं चाहता।

जब तक डॉ. आंबेडकर जी को पढ़ेंगे नहीं तब तक अपना विकास नहीं कर सकते। आपकी इस देश में क्या स्थिति है, आपकी इस देश में यह स्थिति क्यों हैं, आप इतने पिछड़े क्यों हो, आपके अधिकार क्या हैं, आप आरक्षण होते हुए भी राजनैतिक रूप से पिछड़े हुए हो, आप बड़े व्यवसायी क्यों नहीं बन पाए, इन सब प्रश्नों का उत्तर डॉ. आंबेडकर जी देते हैं। डॉ. आंबेडकर जी ने आखिर पुस्तकें क्यों लिखी ? वह बड़े वकील थे। समाज के भविष्य के लिए पुस्तकें लिखने की जगह वकालत से धन कमा सकते थे। डॉ. आंबेडकर जी ने आपके लिए दुनिया का सबसे लंबा संविधान लिखा। वर्षों तक संविधान लिखने की जगह वह बड़े व्यापारियों के मुकद्दमें लड़ सकते थे और धन कमा सकते थे। कानून मंत्री रहते हुए उन्होंने महिलाओं के अधिकारों के लिए इस्तीफा दे दिया। वह चाहते तो इस्तीफा नहीं देते और मंत्रिपद से चिपके रहते। इतना सब बाबा साहिब डॉ. आंबेडकर जी कर गए पर जो उनकी मेहनत का आज फल भोग रहे हैं वह उन्हें पढ़ना भी नहीं चाहते ? बाबा साहेब तो कभी धन के पीछे नहीं भागे पर आज लोग आँख बंद करके धन के पीछे भाग रहे हैं।

तो भले ही यह नोटबंदी कुछ दिनों की हो। लोगों को अपनी-अपनी स्थिति के बारे में सोचना चाहिए। उन्हयें यह आंकलन लगना चाहिए कि उन्हें किन व्यसनों की लत (एडिक्शन) है। उन्हें यह सोचना चाहिए कि उनका बौद्धिक विकास कितना हुआ है। डॉक्टर या इंजिनियर या कोई अफसर बनने का अर्थ यह नहीं कि किसी में बौद्धिकता भी हो। कोई एम एन सी में मोटी आय पर लगा हो या किसी व्यवसाय से धन कमा रहा हो या नेता बन गया हो, बौद्धिकता की कमी सभी में है। जिनके भरोसे आप जी रहे थे उन्होंने एक पल में ही आपको लाइन में खड़ा कर दिया। क्योंकि आपने सोचना और समझना बंद कर दिया और अपने को चंद लोगों का गुलाम बना दिया।तो देखिए कि उन चंद लोगों ने आपको अपने ही धन के लिए लाइन में लगवा दिया।  आपको जिस धन में स्वतंत्रता नज़र आती थी उसी धन के लिए आप कितने गुलाम हैं, यह आपको एक पल में बता दिया गया। इसलिए आप भविष्य के लिए सतर्क रहें। अपनी लातें छोड़ें। अपनी बौद्धिकता का विकास करके बुद्धिजीवी बनना अपने जीवन का लक्ष्य बनाएं।

आप सरकारी नौकर हैं तो रोजाना श्रम करनेवालों के भविष्य की सोचें। आप गैरसरकारी नौकरी में हैं तो व्यवसाय करने की सोचें। आप व्यवसाय में हैं तो अपने यहाँ काम करनेवालों के परिवार की शिक्षा और उनकी उन्नति की सोचें। आप नेता या मंत्री हैं तो केवल-और-केवल समाज के भले की ही सोचें। याद रखिए कि किसी भी समाज में मनुष्य को वही प्राप्त होता है जिस समाज  का हिस्सा वह स्वयं होता है। आप समाज को मजबूत बनाइए तो समाज आपको कमजोर पड़ने ही नहीं देगा। आप समाज को धनी बनिए तो समाज में आपको धन की कमी महसूस ही नहीं होगी। आप समाज को ईमानदार बनाइए तो आपके साथ बेईमानी होगी ही नहीं। यदि हम देंगे तो हमें मिलेगा भी। यदि हम सिर्फ लेने की सोचेंगे तो हमसे छीना भी जा सकता है। आज यही स्थिति है कि जिन्होंने सिर्फ लेने की ही सोची उन्हें छिन जाने का डर है। यदि लेने वालों ने बांटना सीख लिया होता तो आज किसी को उनसे छीनने की जरुरत नहीं पड़ती।

डॉ. आंबेडकर जी ने सदा, देश और दुनिया को अपना दिया। उन्होंने जो कड़े परिश्रम से विद्या प्राप्त की उससे भारत के टूटे और भिखरे हुए समाज को मजबूत और एकजुट किया। अपनी शिक्षा और कार्यों के अनुभव पर उन्होंने संविधान की रचना की और देश ही नहीं बल्कि विश्व को एक श्रेष्ठ संविधान दिया। आज डॉ. आंबेडकर जी को इतना सम्मान दिया जाता है क्योंकि उन्होंने समाज को सिर्फ और सिर्फ दिया। अपनी शिक्षा को भी उन्होंने केवल निजी स्वार्थ के लिए ही सीमित नहीं रखा बल्कि उसमें अपने अन्य अनुभव जोड़ कर उन्होंने श्रेष्ट ग्रंथों की रचना की। वह केवल और केवल समाज को देना चाहते थे। समाज को अपना वह दान वह न केवल अपने जीवनकाल तक ही सीमित रखना चाहते थे बल्कि उसके बाद भी। इसलिए उन्होंने पुस्तकें लिखी, अपने लेखों को पुस्तकों के रूप में प्रकाशित करवाया और ऐसे विधान (कानून) बनाए जिनसे उन लोगों को शिक्षा प्राप्ति का मौक़ा मिल पाया जिनको पीढ़ियों से शिक्षा से वंचित रखा गया था।

परंतु आज जिन लोगों को डॉ. आंबेडकर जी की तपस्या से फल प्राप्त हो रहे हैं वह केवल अपने निजी स्वार्थों तक ही सिमित रहना चाहते हैं। वह अपने फलों को बांटना नहीं चाहते। वह फल देनेवाली व्यवस्था को उत्पन्न करनेवाले बाबा साहिब के उन विचारों को नहीं जानना चाहते जो कि उन्होंने उन पुस्तकों के रूप में अपने आनेवाली पीढ़ी के लिए रखे थे जो उनके जीवनकाल के बाद पैदा हुए। उनके जीवनकाल के बाद वाली पीढ़ी उनके बनाए आरक्षण और संवैधानिक समता से प्राप्त हुए अवसरों को तो चाहती है पर उनकी शिक्षा को ग्रहण करना नहीं चाहती। वह डॉ. आंबेडकर जी से ऋण तो ले रही है पर न ही तो समाज का भला कर रही जिससे की डॉ. आंबेडकर जी का ऋण चुका सके और न ही डॉ. आंबेडकर जी को पढ़ रही जिससे कि वह अपनी कृतज्ञता प्रदर्शित कर सके।

New Delhi: The COVID-19 pandemic highlighted the importance of repairing our ailing healthcare infrastructure. As a result, according to the Economic Survey of 2022, India's public expenditure on healthcare was 2.1 per cent of GDP in 2022, up from 1.8 per cent in 2021 and 1.3 per cent in 2020. Countries like France, Japan, and Canada spend 10 per cent of their GDP on public healthcare, while neighbours like Pakistan and Bangladesh surpass us by spending over 3 per cent.

But despite these splendid achievements, there are incendiary challenges as well. The pandemic has put the focus back on primary healthcare systems. For centuries, primary healthcare in India has been confined to the neighbourhood family doctor. From baby boomers to generation X, a nearby family doctor was always around. But for gen Z (millennials), that has been replaced by big hospital chains. Healthcare professionals have a firm belief that this phenomenon of OPDs in big hospitals is becoming counterproductive due to a lack of diligence and scarcity of time. It is not only leading to a delayed response to a disease but is also the reason behind high out-of-pocket expenses (OOPE).

India stands among the highest in the world in terms of OOPE, with a whopping 63 per cent of the points. High OOPE on health is impoverishing some 55 million Indians annually, with over 17 per cent of households incurring catastrophic levels of health expenditures every year, according to a World Health Organisation (WHO) report from March 2022. This can lead to patients drifting toward alternative remedies that may not be scientific at all. ETHealthword takes a look at some of the most critical areas of the Union Budget, covering the major heads of expenditure over the past couple of years.

Overall Union Health Budget trends since 2019 (pre-COVID vs post-COVID)

The Fifteenth Finance Commission, in its voluminous report titled, ‘Finance commission in COVID times—Report for 2021–26’, specifically identified critical areas in healthcare that need the government’s attention, including low investment, sharp inter-state variations in the availability of health infrastructure and outcomes, supply-side problems of doctors, paramedics, hospitals, and an inadequate number of healthcare centres like primary healthcare centres (PHCs), subcentres, and community health centres (CHCs). To achieve these targets, India needs to burn the candle at both ends in terms of increasing the overall capital expenditure in healthcare but also needs to have a focused strategy to meet the specific challenges as well.

The National Health Policy 2017 (NHP 2017) has proposed the ramping up of public health expenditures to 2.5 per cent of GDP by 2025. In 2019-20, the centre’s expenditure on healthcare was Rs 64,258 crore, which is around 1.4 per cent of the country’s gross domestic product (GDP). The percentage of healthcare in GDP rose to 1.8 per cent, with the centre spending Rs 80,694 crores in the budget for 2020–21. Following that, the GDP increased by 0.3 per cent in the years 2021–22, to Rs 86,001 crores (accounting for 2.1 per cent of the GDP). The 2022–23 budget estimate (BE) stands at around Rs 86,201 crore. The annualised change in budget expenditure is approximately 8.94 per cent (from actuals 2019–20 to BE 2022-23). The report of the Finance Commission recommends unconditional funds of Rs 1 lakh crore for the health sector between 2021 and 2026.

Lack of attention towards primary healthcare

Primary healthcare acts as a foremost protection layer against the spread of diseases, both on an individual and community level. Although in India, health is a state subject and the responsibility for primary healthcare vests with the states. Since it is a unitary federal system, the central government also tries to focus on primary healthcare through its two initiatives: the National Health Mission (NHM) and the Pradhan Mantri Aayushman Bharat Health Infrastructure Mission (PM ABHIM). There is a three-tiered system in primary healthcare, including sub-centres (SCs), primary health centres (PHCs), and community health centres (CHCs). According to the findings of the Rural Health Statistics, India still has a long way to go before standardised SCs, PHCs, and CHCs become a reality. The centre has set a goal in the 2017-18 Union Budget to convert existing SCs and CHCs into 150, 000 health and wellness centres (HWCs) by 2022. As of now, we are stuck at around 90,000 HWCs. The Fifteenth Finance Commission has recommended intensifying the focus on primary healthcare by increasing the budgetary allocation to Rs 70,000 crore at the primary level between 2021 and 2026.

Combining the budget of both NHM and PM ABHIM for years 2022–2023, the budget estimates for which stand at Rs 42,846 crore, which is far less than what the Finance Commission envisaged. The 2021–22 budget’s revised estimates for both schemes combined were Rs 35,457 crore, an overall 82 per cent lesser than 2022–23 BE. Before 2021, there was no PM ABHIM; the NHM solely focused on primary healthcare. The actual expenditures for the NHM in 2020 and 2019 were Rs 37,080 crore and Rs 31,745 crore, respectively.

High out-of-pocket expenditures vs lower penetration of health insurance




India’s out-of-pocket expenditure (OOPE) is considerably high if we compare it with the rest of the world. Although it has come down substantially, as per the National Health Estimates (NHE) 2018–19, from about 64.2 per cent in 2013–14 to 48.2 per cent in 2018–19, the road is still ahead to achieving the World Bank’s global average of around 18.1 per cent. According to the WHO’s India health system review report, high OOPE impoverishes around 55 million Indians annually. According to the Economic Survey 2020–21, out-of-pocket healthcare costs in India account for 60 per cent of all public health spending, which is one of the highest in the world.

Intensified capital expenditure on public health insurance can be one way to take on this problem. The penetration of public health insurance in India is very low. According to the NITI Aayog report 2021 titled ‘Health Insurance of India’s Missing Middle’, a population of around 40 crore individuals is still devoid of any health insurance financial protection. The Indian government, through its Ayushman Bharat—Pradhan Mantri Jan Arogya Yojana (AB-PMJAY), is trying to proliferate public health insurance in India.

The budgetary allocation for the AB-PMJAY scheme has consistently been above Rs 6,000 crore net since it replaced the Rashtriya Swasthya Suraksha Bima Yojana in 2019. However, due to lower penetration of public health insurance and poorer utilisation of funds, the scheme gets downsized every year by 50 per cent in the revised estimates. The Fifteenth Finance Commission's report on Ayushman Bharat (2019) anticipated the demand for and costs associated with PMJAY over the following five years. According to the report, the overall PMJAY costs for 2019 (centre and states) might range from Rs 28,000 crore to Rs 74,000 crore.

Human resources and government investment in institutions
The targets set by the WHO in terms of the doctor-to-patient ratio are 1 doctor for every 1000 patients. The Government of India (GoI) controversially claimed in parliament in December 2021 that India's doctor-to-patient ratio had significantly improved to 1 doctor for every 834 patients by including 5.65 lakh AYUSH doctors alongside allopathic practitioners. Although figures from 2019 suggest that India’s doctor-to-patient ratio stood at 1:1511.

Entrusted with the objective of correcting regional imbalances in the availability of affordable and reliable tertiary healthcare services, the Pradhan Mantri Swasthya Suraksha Yojana (PMSSY), introduced in 2003, supervises the formation of institutions like AIIMS and upgrades certain state government hospitals. The scheme covers 20 new AIIMS and 71 state government hospitals. PMSSY has been given Rs 10,000 crore for 2022–2023. This is a 35 per cent rise from the revised estimates for 2021–2022 (Rs 7,400 crore). In 2020–21, the revised estimate for PMSSY was Rs 7,517 crore, 25 per cent higher than the budget estimate of Rs 6,020 crore. Apart from this, there is an overall nine per cent increase in the budget allocation to autonomous institutions like AIIMS, New Delhi, the Postgraduate Institute of Medical Education and Research, Chandigarh, and the Jawaharlal Institute of Postgraduate Medical Education and Research, Puducherry, from 2020–21 actuals (Rs 12,197 crore) to 2022–23 budget estimates (Rs 15,200 crore).

National AIDS and STD Control Programme, Family Welfare Scheme, and COVID-19




Beginning in the form of ‘sero-survillance’ in 1985, India’s response to diseases like AIDS became a full-blown measure with the passage of the HIV and AIDS (Prevention and Control) Bill, 2017, and the launch of the ‘Test and Treat’ policy for HIV patients in April 2017. The Union Cabinet continued NACP Phase V, a Central Sector Scheme, from April 1, 2021, to March 31, 2026, with an outlay of Rs 15471.94 crore. The actual expenditure on the NACP programme in 2020–21 was Rs 2,815 crore. A year later, it was increased to Rs 2,350 crore, and in 2022–23, the scheme was allocated Rs 2,623 crore. The rate of change from 2020 to 2022 was 11.6 per cent positive.

The Family Welfare Scheme also gets major attention from the centre. It is an umbrella scheme covering several initiatives run by the government, ie, the Urban Family Welfare Programme, the urban revamping scheme, the green card scheme, the rural family welfare centre etc. Budgetary provisions for Family Welfare Schemes have not changed significantly since actual spending in 2020-21.

In 2020–21, the actual spent was Rs 2,815 crore; thereafter, in 2021–22, it was around Rs 2,350 crore; and finally, in the previous budget, the BE was set at Rs 2, 623 crore.

The central government allocated around Rs 11, 941 crore for COVID-19-related initiatives, which witnessed an over Rs 5000 crore increment to about Rs 16, 545 crore. As the pandemic started to slowly fade out, the budgetary allocation came heftily down to Rs 226 crore in the Union Budget 2022–23.

This year there may not be any major attribution of funds towards the COVID-19 counter-initiatives; experts believe that the government is going to use these funds for some other major purposes.

Govt may bring new law to ban tobacco products: Min

The state government had in 2013 imposed a ban on tobacco and gutka products, using a temporary provision under the Food Safety and Standards Act, 2006. The ban was subsequently extended every year through notifications. Recently, the Madras high court had quashed the temporary notification.

Coimbatore: Health minister Ma Subramanian on Tuesday said the state government would amend the Food Safety and Standards Act, 2006, or pass a new legislation, if required, to continue the ban on tobacco products.

The minister, who was in the city to attend the white coat ceremony at the Coimbatore Medical College and Hospital and the ESIC Hospital, and administer Hippocratic oath to the medical students there, also appealed the shopkeepers in the state not to sell tobacco products, as they were posing serious health risks to people.

“Shopkeepers should ponder over whether to sell something that will endanger people’s health and use the proceeds from the same for their livelihood. When there are many other items in the shop, why indulge in selling tobacco products? I appeal them to stay away from selling the same,” he said.

A M Vikrama Raja, president, Federation of Tamil Nadu Traders Association, had recently sought the state government’s response to the court order and whether tobacco products could be sold in shops.

“The state government will appeal against the high court order. We will amend the act or pass a new legislation, if required, to continue the ban on tobacco products,” Ma Subramanian clarified.

He said chief minister M K Stalin would open 500 health and wellness centres across the state in February. While Coimbatore has been allotted 72 health and wellness centres, 50-60 of them would be inaugurated then, he said. At least 64 of the 72 health and wellness centres would be in the city corporation limits.

The minister said the number of outpatients in the government hospitals had doubled now compared to 2018-19. “To provide better care for the increasing number of patients, vacancies are being filled up. The chief minister will issue 4,308 appointment orders to fill various posts in the government hospitals at an event to be held in Chennai on February 3.”

Subramanian said efforts were on to make Tamil Nadu a tuberculosis free state by 2025.

The minister dedicated medical equipment worth 2.73 crore to various hospitals in the state under the ‘Innuyir Kaapoom-Nammai Kaakkum 48’ scheme on the day. The ESIC Hospital got medical equipment worth 56 lakh. He said 1, 41, 923 people had benefited from the scheme so far. The state government has spent 125.42 crore

Union Budget 2023-24: Budget not as per expectations

Healthcare stakeholders are dismayed that there was no increase in the budget allocation, no revision/reduction in GST, no tax exemption and missing out on the crucial aspect of universal health coverage.

New Delhi: Healthcare leaders and experts shared their views on the Union Budget 2023-24. Informing that the new initiative like setting up 157 new nursing colleges, centres of excellence in AI, strong emphasis to promote innovation and R&D, PPP in healthcare and MedTech specialised workforce, and mission to eliminate sickle cell anaemia is a step in the right direction.

On the other hand, industry stakeholders are of the view that success will be determined by how well it is implemented. At the same time, the healthcare stakeholders are dismayed that there was no increase in the budget allocation, no revision/reduction in GST, no tax exemption and missing out on the crucial aspect of universal health coverage.

Sharing his views on the Union Budget announcement, Gautam Khanna, CEO, PD Hinduja Hospital & Chairman FICCI Health Services, mentioned, “Budget 2023-24 focused on enhancing India’s capabilities and resources through increased manpower, R&D, and PPP in the healthcare sector. Setting up 157 new nursing colleges is welcome, in view of the severe shortage of nurses in the country. However, the current state of existing nursing colleges must be evaluated for upgradation & better job opportunity for nurses to be identified to curb international migration.

The idea of setting up a centre of excellence in AI for health along with the strengthened impetus towards medical education will accelerate the development of new-age, technology-driven medical solutions for better disease management and encourage start-ups to come up with innovative solutions in the healthcare delivery space. Creating awareness of prevention and early screening is not only essential in eliminating sickle cell anaemia but would also be a great stepping stone for similar diseases, however, its success will depend on effective implementation.

The increased focus on encouraging medical education and PPP will aid the industry’s growth. The budgeted increase in healthcare expenditure of 15 per cent does not seem enough to tide over the current challenges of the upgradation of infrastructure and providing accessibility and affordability for quality healthcare in the country.
  
We are looking forward to clear indications of the steps to be implemented for healthcare infrastructure development and moving closer towards universal health coverage with increased expenditure as a per cent of GDP. It would have been better if there were tax exemptions for healthcare, which is vital to reduce healthcare expenses & out-of-pocket spending.”

Skilling of healthcare nurses and allied workers has been a long-standing gap, hindering healthcare delivery expansion, and the union government has taken a step in the right direction to address this. A significant increase in nursing and medical colleges, a strong emphasis to promote innovation and R&D, PPP in healthcare and MedTech specialised workforce will lay a stronger roadmap for the future.

Subramanyam continued,
“The initiatives announced to strengthen the digital healthcare infrastructure will prove to be beneficial for the healthcare sector in the years to come.
The mission to eliminate sickle cell anaemia by 2047, income tax rebates and disposable income to be channelled to essential items - healthcare being one, are laudable efforts undertaken by the government.

However, the long-standing requirements of the sector to increase healthcare spending as GDP per cent to 2.5 per cent, custom duty reduction to balance demand with supply and the nascent healthcare manufacturing base in equipment scheme, streamlining embedded indirect taxes like GST in healthcare making a smooth flow of credit difficult across the value chain and low-cost financing schemes to strengthen healthcare infrastructure, primary care and improved working capital has not been addressed. In the last couple of years, a big positive is the partnership spirit with the private sector in healthcare and coherent initiatives undertaken to take these goals towards reality for a healthier prosperous India. NATHEALTH looks at working closely with governments at all levels to take this aspirational healthcare agenda forward towards Arogya Bharat and beyond.

Emphasising the need for universal healthcare and the ever-rising disease burden in the country, Dr BS AjaiKumar, Executive Chairman, Healthcare Global Enterprises Limited opined, “The budget has again missed out on addressing universal healthcare which is very disappointing. The increase in budgetary allocation to this sector is yet off the desired mark, given the severity of the issues and challenges on the ground like high infant mortality rates, alarming out-of-pocket expenditures, and vulnerable public health infrastructure in rural areas and deprived regions. Given that the government has increased tax on certain forms of cigarettes by 16 per cent, it is important that this extra tax collection should be earmarked for healthcare, particularly for cancer care, as the victims of tobacco use are cancer patients.

“On the bright side, the announcement of opening new nursing colleges is a laudable move given the critical importance of the nursing professionals but such announcements should precede a summary of the earlier budget announcements and respective implementations on the ground. This context will lend more clarity and transparency to the budget announcements.”

“The renewed focus on the genetic blood disorder of sickle cell anaemia is a key step, but its implementation will hold the key to its sustainable success. It may be recalled that earlier budgets had focused on the ‘anaemia Mukt Bharat’ mission, pledging to treat anaemia through testing, point of care, and supply of fortified foods. A preface on the outcome of that scheme would have been apt while announcing the new measure.

The impetus provided for public-private collaboration is welcome, both through the ICMR lab initiative as also through the pharma centres of excellence. However, there is a pressing need to invest in tech-enabled systems using predictive genomics to strengthen India’s prevention, surveillance, and response capacities to pandemics and epidemics. I was also expecting a rationalisation of tax and duty structures around life-saving drugs and emergency treatments, revision of GST slabs, and SOPs for healthcare equipment manufacturers. High time, India makes health a top priority and extends all key subsidies and benefits to the sector to make healthcare accessible and affordable to one and all,” concluded Dr AjaiKumar.

Dilip Jose, MD & CEO, Manipal Health Enterprises Pvt Ltd, stated, ”It is a progressive budget with a focus on infrastructure development, which should in turn spur job creation. The steps to encourage environment-friendly consumption and businesses are very welcome too. The budget is also mindful of the expectations of the youth and the middle class.”

Adding to this Commander Navneet Bali, Regional Director Narayana Health-North said Union Budget 2023 looks progressive and inclusive for the healthcare sector. “The government has taken a holistic approach by focusing on the ‘Sapt Rishi’ model and our sector is aligned with all the seven pillars mentioned by the Finance Minister in her speech. For example, the healthcare sector plays a crucial role in inclusive development and reaching the last mile. We are glad to know that ICMR labs will be made available for research by public & private medical college faculty and private sector R&D teams. We were expecting some measures on capacity building in the sector and it is promising to note that the government has announced setting up 157 new nursing colleges. This would enhance our capacity and fill the gap in terms of human resources. The proposed mission to eliminate sickle cell anaemia by 2047 is a very positive step. The programme for research & innovation in pharmaceuticals is a well-appreciated need of the hour.”

Sugandh Ahluwalia, Chief Strategy Officer, Indian Spinal Injuries Centre said, “Provisions for the healthcare sector, announced by Union Finance Minister Nirmala Sitharaman, look promising. The creation of 157 nursing colleges, in combination with the existing 157 medical colleges established since 2014, would create a huge capacity for healthcare providers. Another important takeaway is a push for a public-private partnership by making available facilities in select ICMR labs for research by public and private medical institutions. As we were expecting some incentives in medical value tourism (MVT), the overall thrust to promote tourism, thereby extending facilities to overseas tourists, would also benefit medical tourism in the country. With the announced measures, the healthcare industry is hopeful to conduct more interdisciplinary research and develop cutting-edge applications and scalable problem solutions.

Dr Anil Krishna, Chairman & Managing Director, Medicover Hospital stated, “A bold budget that lays down a 25-year roadmap for India to become a truly successful economy. The announcement regarding the increase in healthcare-related expenditure with an increase of almost 10.91 per cent over the revised estimate of 2021-22 and 2022-23 is truly a massive move. It shows that the government has learned from the pandemic as they plan on spending Rs 39,44,909 crore in 2022-23. Furthermore, the actions made for collaborative public health management demonstrate that the government is finally looking at healthcare R&D through expanding facilities in select ICMR (Indian Council of Medical Research) labs. These will be made available for research by academics from public and private medical colleges, as well as R&D teams from the private sector, to encourage joint research and innovation. All in all, it is a very well thought out budget that will have a positive impact on the digital and health infrastructure of the country.”

Quick comments

 Some quick comments on the health budget as below

1. The bureaucrats and members of parliament continue to assure for themselves a very robust share of the health budget for their own privilege. This Rs 6066.24 crores for the 42.38 lakh beneficiaries amounts to Rs 14314 crore per beneficiary contribution by the Union budget in sharp contrast to Rs 86737 crores (92803 crores - 6066 crores) which woks out to a mere Rs  620 per capita. And even if we add the state health budgets which would add up to is about Rs 340,000 crores then the per capita expenditure works out to Rs 2460 per capita and this being 5.8 times less than our honourable bureaucrats and MPs ge ( Its time JSA made this issue of inequality between the electors and the elected and public servants a major highlight ) .
2. Overall too little is allocated to healthcare. The policy mandate is 2.5% of GDP for healthcare (ofcourse excluding water and sanitation which the Eco Survey made us believe that we are spending 2.2% of GDP on health) which is todayRs 680000 crores. Of this the NHP says the Centre should contribute 40%. Accordingly the Centre's health budget should be Rs 270000 crores. This means that the allocation deficit by the Centre is to the tune of Rs 178000 crores. So health clearly not a priority in the budget despite benchmarking from their own health policy
3. The budget structure is becoming increasingly less transparent. The 2021-22 budget information includes much more details than the subsequent years. Thus NHM is now a single entry in the budget in contrast to 2021-22 when NHM was disaggregated by various programs. Earlier years provided even more disaggregated information for example by each disease program, family welfare activities, breakdown of primary healthcare and hospital expenditures etc..
4. We see significant increases in budgets of technology driven and infrastructure related allocations - NDHM, PMABHIM etc as well as PMJAY probably because these promote private sector partnerships or privatization
5. The NHM allocations in the Expenditure statement dont seem to be complete. As per the schemewise table in Expenditure profile docum,ent NHM was Rs 37800 in 2022-23 and in 2023 -24 is down to an allocation of Rs 6785 crores
6. PMJAY is a problematic budget item. It receives a huge allocation but spending is usually half of that each year. For instance in 2021-22 the budget was Rs 7500 crores but actual spending was only Rs 3116 crores. And we know from NITI Aayog data that 78% of the PMJAY funds flow into private hospitals.

BUDGET 2023-2024: A FRAUD ON THE WOMEN OF THE COUNTRY

The All India Democratic Women’s Association (AIDWA) sees the Modi Government’s last full budget before the general elections of 2024, as an attempt to hoodwink women and working people of the country in order to promote the agenda of big corporate houses. The narrative unleashed by the government in the name of the budget, is designed to perpetrate false hoods and show that all is well, and people are becoming prosperous. However, we need to contrast the PR of the government with the prevailing social reality.

The presentation of the budget should be seen in the context of the widening inequality and deepening economic distress. As the Global Inequality Report, 2022, shows 1 percent of the  population, namely the capitalist cronies of the BJP government, control more than 40 percent of the wealth of the country, whereas the bottom 50 percent control only 3 percent and are scrambling for survival. Women and their families are facing a severe employment crisis, but the budget is oblivious to this reality. Instead, it provides minimal tax relief to the salaried class, and ignores the  plight of the self-employed and a majority of those in informal labour. This is likely to exacerbate rather than reduce the inequality within the country.

A closer look at the budget shows us that the Government has actually cut back on its expenditure and the size of the government expenditure has in fact become smaller. In nominal terms, the proportion of the Union budget to GDP has come down from 15.3 percent(revised)   in 2022-2023, to 14.9 percent (budget estimates) in 2023-24. However, if we account for the current inflation rate, there is in fact a reduction in the total budgetary expenditure of the Union government.

The PM and Finance Minister have touted this budget as a pro-women budget, but it's figures tell a totally different story. The total proportion of the Gender Budget as proportion of GDP has increased from 0.71 percent (RE) 2022-2023 to 0.73 percent (BE), 2023-24. But if we account for inflation, the real value of these allocations is less by about 3 percent which is lower than last year. Further, most of the nominal increases are because of one scheme, the  PM Awas Yojana, which has got a big fillip in this budget. But this increase is notional, because the Union Government’s record of release and utilization of funds in this scheme is abysmal, and most states received only half of their approved budgets in 2022.

As far as the Ministry of Women and Child Development is concerned, its total expenditure is only about 0.05 percent of the entire expenditure budget. About 80 percent of this is for the Saksham Anganwadi POSHAN 2.0 scheme. The budget for schemes addressing safety issues and violence against women have minimal budgetary increases. Further, there are virtually no provisions for sports women, whose safety has become a major concern in recent times.

The main emphasis of the budget is on mobilizing women’s savings and linking them with corporate supply chains. The Finance Minister touted that the Deen Dayal National Rural Livelihood Mission has 81 lakh savings groups which will be upgraded and made into large enterprises. However, the credit provided to them will be largely through private players and there are no announcements for low interest credit lines for women. Further, the new savings scheme announced for women, Mahila Samaan Saving Patra, is only going to cover only a handful of women who can afford to save. Thus, the financial schemes announced by the government will do little to address the problem of increasing indebtedness of women. The root cause of this debt trap is both, lack of livelihood and the high interest rates charged by the MFIs and private Small Finance Banks. Rather it will put the savings of women at the disposal of these private lending agencies, whose share is growing in financial markets, because of the policies of the Modi government.

Further, employment generation schemes like MGNREGS, have experienced cut backs to the tune of 33 percent. In fact, such cutbacks are a direct attack on women-oriented schemes. As expected, there is no announcement for recognition of the rights of scheme workers, or for enhancing their remuneration. The allocations under the National Assistance Programme (including widow pension), and schemes for welfare of scheduled tribes and scheduled castes have seen nominal and no substantive increases.

Despite making tall claims about providing free food to 80 Crore families and extending the term of the PM Garib Kalyan Yojana for one year, the food subsidy has come down by 31 percent, in nominal terms. It is also one third less than the actual expenditure in 2021-2022. Further, there has been a decrease, even in nominal terms in the National Midday Meal programme and the allocations for the Anganwadi programme are almost the same as the revised budget for last year. This allocation can also be considered a cutback in real terms, when we account for inflation.

Expenditures on education have seen small budgetary increases in nominal terms. However, the most worrying trend in educational expenditure is the emphasis on digital education, non-restoration of Rajiv Gandhi and Maulana Azad Fellowships, and the emphasis on student loans  as a financing model for education. All these will have an adverse impact on women/girls education. Expenditures on the Health Mission ( including maternal and child health) have seen nominal decreases. The only health spending with some substantive increase is Ayushman Bharat, which is dominated by private sector companies. Thus, we see that the government is actually making massive cuts in social welfare expenditure, rather than providing a model for ‘inclusive development’.

The All India Democratic Women’s Association sees this budget as an exercise to subsidise big corporate houses, at the expense of women and their families. It calls upon all its units and the democratic forces to hold protests and expose the fraud that the government is perpetrating in the name of an ‘inclusive budget’ and work for the defeat of this draconian and pro-corporate Government in 2024.
